Photoshop Color Problem? - White's been replaced with a yellow everywhere??

I know the FAQ says don't post color problems in here, but the link in there's dead, so, I'm going to have to ask you anyway.
I opened this file ( http://nxc.ereda.co.uk/photoshop/ticket-template-2stub.psd ) the other day in photoshop to design some tickets for something. But... it opened with white being replaced by some skanky yellow colour ( http://nxc.ereda.co.uk/photoshop/yellowness.gif).
Now, every document I open has this skanky yellow as default, ( http://nxc.ereda.co.uk/photoshop/proofing.gif - this is a different document in RGB colour) and every time i select white in the RGB color picker, i get this little symbol popping up.

But, when I proof the colors using my monitors default color profile, white is actually white again.( http://nxc.ereda.co.uk/photoshop/whiteness.gif )
I've tried picking all the color profiles just about everywhere I can, and it still comes out skanky yellow. now every document I open has yellow as it's white, unless I keep 'Proofing' on all the time?
Can someone please tell me what i've done wrong and how to fix it?
